Cost:

Services are provided to all persons regardless of income.

All charges are determined by a sliding fee scale that is based on the individual’s household income.

Clients are required to bring verification of income when presenting for appointments.

State sponsored: Medicaid, HMO’s, Clinic Plans and QHP’s are accepted.

Appointments:

All services require an appointment. Appointments can be made by calling: Monday thru Friday 9:00 am to 11:30 am and 1:00 pm to 4:00 pm.
